Our clinical staff, which is available to our patients 24 hours a day, 7 days a week, is designed to meet a wide range of patient needs. Our nurses monitor medical conditions, administer medications, draw blood for testing, and provide IV treatments. Physical therapists ensure that our patients receive the bodily conditioning and care that they need to improve physical function, utilizing therapeutic exercises, gait training, and training with prosthetics.

What are they and how do they work?
To support our integrative approach to home health, our proprietary, evidence-based programs are designed to provide custom care plans for a variety of diagnoses to best serve our patients and their families.
These programs work by taking patients from the first steps of stabilizing their condition through to understanding their disease and helping patients reach individual goals. We confirm medications and help navigate medication changes, in addition to explaining the role each medication plays in the patient’s treatment.
We also assess the safety of patients’ homes and ensure access to adaptive equipment if required. By the end of the program, patients are taught how to monitor their conditions and recognize changes that may impact their condition.
